GERRIC Student Programs Asthma, Anaphylaxis and Medication Procedures

Does your child have asthma, anaphylaxis or medications we should know about for the student programs? We require you to follow the below procedures to ensure the safety of your child during the programs. Without the appropriate documentation, your child will not be permitted to join the programs for safety reasons. 

For the July 2019 GERRIC Student Programs, please send completed plans to gerric@unsw.edu.au by the 1st July, 2019. Thank you. 

Asthma

  1. Develop an Asthma Action Plan (see here (PDF) for the approved format) with your Doctor.
  2. Send this plan to gerric@unsw.edu.au with the subject line "Your Child's Name - Asthma Action Plan". 

Please ensure there is a photo attached to the plan, our staff need the photos to identify students quickly. Also please only send in doctor approved plans. 

Anaphylaxis

  1. Develop an ASCIA Anaphylaxis Action Plan (see here for the approved format) with your Doctor.
  2. Send this plan to gerric@unsw.edu.au with the subject line "You Child's Name - Anaphylaxis Action Plan". 

Please ensure there is a photo attached to the plan, our staff need the photos to identify students quickly. Also please only send in doctor approved plans.

Medications

  1. Complete this form (PDF) [265 Kb]
  2. Send the form to gerric@unsw.edu.au with the subject line "Your Child's Name - Medication Form".

Please ensure there is a photo attached to the plan, our staff need the photos to identify students quickly. You may also bring a physical copy of the form in to the programs. GERRIC staff will not administer medications without a completed form.
